Permanent Case Management Assistant

We are seeking a dedicated Case Management Assistant to join our team in Richardson, TX. In this role, you will coordinate and facilitate Team Conference meetings, gather and maintain case management documentation including referrals, admissions, orders, and schedules, and effectively communicate daily assignment updates. You will also complete pre- and post-discharge activities to ensure patients, families, and clinicians are well-prepared, and assist with quality improvement and performance improvement data collection.

This is a permanent, staff position offering an average pay of $23 per hour. Schedule flexibility is required, including weekdays, weekends, evenings, or night shifts as needed, as well as availability during holidays.

Qualifications include a high school diploma or equivalent (preferred), proficiency in Microsoft Office, strong typing skills, and knowledge of medical terminology or insurance/payor systems (preferred).
