Description
We are seeking an experienced Registered Nurse Case Manager to join our team in Attleboro, Massachusetts. In this role, you will collaborate with patients, families, social workers, physicians, and the interdisciplinary team to assess, coordinate, and facilitate patient progression through the continuum of care. Your focus will be on ensuring efficient, cost-effective transitions from pre-admission through post-discharge, utilizing early assessment and timely communication.
This is a permanent staff position offering an average pay of $55 per hour. You will work in an acute care setting, applying your skills in utilization review and critical thinking to manage patient status and discharge planning. The schedule is full-time, and you will be expected to travel across the system as needed.
To qualify, you must have a current Massachusetts RN license and a minimum of 3-5 years of acute care case management experience. A BSN is preferred, and certifications such as CCM or ACMA are a plus. Strong computer skills, including experience with Meditech or Cerner, are beneficial. You will also need excellent communication and analytical abilities to navigate insurance regulations and advocate for patients.
