Permanent Case Manager Registered Nurse

Palmetto Infusion is seeking a dedicated Registered Nurse to join our team as a Permanent Case Manager in West Columbia, SC. In this role, you will coordinate patient care needs between home infusion pharmacy, home health agencies, hospitals, physician offices, and managed care providers for patients receiving infusion therapies in the home setting. You will provide nursing oversight from admission through end of therapy, educate patients and caregivers, and ensure a seamless care experience.

This is a full-time position with a Monday through Friday schedule from 8:00 AM to 5:00 PM. Service areas include Greenville and Columbia, SC, with occasional travel to nearby areas. While pay is not disclosed, we offer competitive compensation and a comprehensive benefits package including medical, dental, vision, 401K, tuition assistance, and paid time off.

To qualify, you must be a graduate of an accredited school of nursing with an active RN license in South Carolina, have at least one year of current clinical nursing experience, and hold BLS certification (or obtain upon hire). Proficiency in MS Office and the ability to work collaboratively in a changing environment are essential.
