Description
Join the VNA of Manchester and Southern New Hampshire as a Home Health and Hospice Liaison. In this role, you will build and maintain strong referral relationships with physicians, hospitals, skilled nursing facilities, and community partners to ensure patients have access to VNA services. You'll promote our programs, educate referral sources, and work collaboratively to transition patients to home care.
This is a full-time, permanent position with a Monday-Friday schedule from 8:00 AM to 4:30 PM. While pay is not disclosed, the role offers a comprehensive benefits package including health, dental, vision, tuition reimbursement, and a 403(b) retirement plan.
Requirements include graduation from an accredited nursing school, 3-5 years of experience in sales, marketing, or homecare, and a valid driver's license. Epic experience is preferred.
