Description
Sinai Hospital in Baltimore, MD is seeking a compassionate and skilled Nurse Navigator to support surgical patients through every phase of their care journey. In this role, you will act as a central point of contact, advocate, and educator, coordinating multidisciplinary care and ensuring each patient receives high-quality, seamless care. You'll develop individualized care plans, provide navigation assistance to access healthcare and community resources, and conduct patient contact in various settings including the hospital, physician offices, and patients' homes.
This full-time position offers a day shift schedule from Monday to Friday, 8:00 AM to 4:30 PM, with an average pay of $51 per hour. You'll collaborate with physicians and the healthcare team to design programs for high-risk patients, participate in community patient navigation development, and support quality improvement through data collection and reporting.
Requirements include a current RN license in Maryland and BLS certification.
