Description
Join CentraCare as a Transitions of Care/Anticoagulation Stewardship Pharmacist in Saint Cloud, MN. In this role, you will ensure safe and continuous medication management as patients move between healthcare settings, collaborating with physicians, nurses, and care coordinators to develop seamless medication plans. You will also serve as a resource for patients on anticoagulation therapy, working with interdisciplinary teams to balance thrombosis prevention and bleeding risk while building this new service.
This full-time position offers 80 hours every two weeks, with day shifts Monday through Friday starting between 7am and 8am, plus occasional weekends and holidays. Pay starts at $59.63 per hour and increases with experience, averaging $75/hr. Qualifications include a Bachelor of Science in Pharmacy or Pharm.D., current Minnesota licensure, and advanced training or hospital experience preferred. You will also serve as a primary preceptor for a PGY-1 pharmacy resident and APPE student rotation.
