Description
Join Encompass Health as a Case Management Assistant in Saint Peters, Missouri. In this role, you will coordinate and facilitate Team Conference meetings, gather and maintain case management documentation such as referrals, admissions, orders, and schedules, and effectively communicate daily assignment updates. You will also complete pre- and post-discharge activities to ensure patients, families, and clinicians are prepared, and assist with quality improvement and performance improvement data collection.
This is a permanent, staff position offering a competitive pay of approximately $22 per hour. The schedule may require flexibility, including weekdays, weekends, evenings, or night shifts, as well as availability during holidays.
