Description
Join our team as a Registered Nurse Case Manager in the Emergency Department at a leading hospital in Attleboro, MA. In this role, you will collaborate with patients, families, social workers, physicians, and the interdisciplinary team to coordinate and facilitate patient progression through the continuum of care. You will assess pre-admission level of care, plan post-hospital discharge needs, and ensure timely communication with all stakeholders.
This is a full-time, 40-hour position with every other weekend rotation. The average pay is $54 per hour, and a $20,000 sign-on bonus is offered. You will work in a fast-paced ED environment, using your critical thinking and problem-solving skills to ensure safe and timely patient transitions.
Requirements include a graduate degree from an accredited school of nursing (BSN/MSN preferred), a current Massachusetts RN license, and 3-5 years of acute care case management experience. CCM or ACMA certification is preferred. You must have excellent communication skills, knowledge of insurance regulations including CMS/Medicaid, and proficiency with computer systems. Experience with Cerner and denials management is a plus.
