Description
We are seeking a compassionate Physical Therapist to join our Home Health and Hospice team serving patients in the East Bay Area, including San Jose. In this role, you will work one-on-one with patients in their homes to help them regain mobility, manage pain, and improve quality of life. You'll collaborate with a multidisciplinary team to develop and implement individualized care plans.
This is a staff/permanent position with a flexible schedule, offering an average pay of $75 per hour. You will conduct in-home evaluations, develop treatment plans, educate patients and caregivers, and document progress. The role requires a current California PT license, a valid driver's license, reliable transportation, and CPR certification. Experience with OASIS and home health or geriatrics is preferred.
