Permanent PT Home Health

We are seeking a compassionate Physical Therapist to join our Home Health and Hospice team serving patients in the East Bay Area, with a focus on North San Jose. In this role, you will work one-on-one with patients in their homes to help them regain mobility, manage pain, and improve quality of life. You'll collaborate with a multidisciplinary team to develop and implement individualized care plans.

This position offers a flexible schedule with full-time or per diem options, and an average pay of $75 per hour. You will conduct in-home evaluations, educate patients and caregivers, and document progress in accordance with regulatory guidelines.

Requirements include a current California PT license, graduation from an accredited program, a valid driver's license, reliable transportation, and CPR certification. Preferred qualifications include OASIS experience and at least one year in home health or geriatrics.
