Description
We are seeking a dedicated Travel Physical Therapist for a Home Health position in Rancho Cordova, California. This role involves providing therapy services in patients' homes, assisted living facilities, senior apartments, and board and care homes. You will treat a variety of diagnoses including ortho, cardio, pulmonary, oncology, neuro, and geriatric conditions, with no pediatrics or hospice. The typical caseload is 3 to 5 patients per day, with visits weighted to account for travel, documentation, and case management.
The schedule is Monday through Friday, 8:30 AM to 5:00 PM, with one weekend per month requested but not required. Holiday work is required but flexible. The pay averages $2,173 per week with guaranteed 40 hours. Mileage is reimbursable per GSA guidelines. You will need a valid California PT license, BLS certification, and an NPI number. Home health experience including OASIS is required, and HomeCare HomeBase (HCHB) experience is preferred. A current driver's license and proof of automobile insurance are also necessary.
Candidates must not have been directly employed by any CommonSpirit facility within the past year. Two supervisory references are required at submission. The orientation process is robust, lasting three weeks with online learning and field precepting. Dress code is business professional with closed-toe shoes.
