Permanent Care Coordinator Registered Nurse

We are seeking a dedicated Registered Nurse to join our Care Coordinator team at Foundation Health Partners in Fairbanks, Alaska. In this role, you will coordinate patient care across the continuum, ensuring seamless transitions and optimal outcomes. You'll work in a supportive environment with a focus on patient-centered care, collaborating with a multidisciplinary team to assess, plan, implement, and evaluate care plans.

This full-time position offers a competitive hourly wage averaging $54/hr, with a range of $40.54 to $67.15 based on experience and education. You'll enjoy a $25,000 retention bonus (DOE), up to $7,500 relocation assistance, and 90 days of temporary housing. Shifts are 12-hour nights, 36 hours per week, with call required.
