You will work closely with patients and families, educating them on complex home care needs and utilizing teach-back methodology to ensure understanding. Your responsibilities include coordinating referrals for skilled home care, medical equipment, and medications, as well as delivering beneficiary notifications such as the Important Message from Medicare.
