Description
Join Grace Medical Center as a Care Coordinator RN in Baltimore, MD. In this full-time role, you will manage a caseload of 50-75 patients, focusing on care coordination for complex chronic conditions. You'll work with primary care interdisciplinary teams to develop and implement patient-centered care plans, reduce hospital readmissions, and improve health outcomes. Day shift hours are 8:30am-5:00pm, with an average pay of $51 per hour.
Responsibilities include comprehensive needs assessments, medication reconciliation, care transitions, and patient education. You'll collaborate with providers, nurses, and community resources to ensure cost-effective, high-quality care. The role requires a BSN, current Maryland RN license, and 3-5 years of acute care nursing experience. Care management certification and ambulatory care experience are preferred.
