Description
As a Care Transition Coordinator RN at Sinai Hospital in Baltimore, you will provide intensive case management to high-risk patients after discharge. This role involves conducting home visits, offering health education, coordinating healthcare needs, and building referral relationships to ensure successful transitions from hospital to home. You'll be a key member of the integrated care team supporting medical respite and Project U-Turn, improving healthcare access for at-risk Maryland residents.
This full-time position offers a day shift schedule from 8:00am to 4:30pm, with pay averaging $51 per hour based on experience. You'll work in the CARE TRANSITIONS department at Sinai Hospital, part of the LifeBridge Health system, which values innovation and community-centered care.
Requirements include a current RN license in Maryland (or compact state), BLS certification from AHA, and at least 3 years of hospital experience (5+ preferred). An ASN is required; BSN is preferred.
